You will need to use Adobe Acrobat Reader. You may type text onto a form, save it to your desktop, and print it. When you are completed, follow the form instructions on how to submit your form to our office.
*For major changes please go to the College offering the major you are interested in. All Colleges will have the "Major Change" form available or you can get the form from the Registrar's Office.
