Applicants randomly selected for verification by either the U.S. Department of Education or the Financial Aid Office must complete a verification worksheet form (Independent or Dependent) and provide any additional documentation requested by the Financial Aid Office. Financial Aid applications are not considered complete and funds will not be disbursed until the verification process is complete.

Income Tax Returns & Verification Worksheet A,B,C
Students chosen for verification must provide a signed copy of federal tax return(s) as listed below. The student's name and social security number should be printed on the top right hand corner of parent's or spouse's tax returns. The Teletax form is not an acceptable document unless the 'confirmation number' is provided.

A fully completed and signed Worksheet A,B,C is required for all students selected for verification. Students, parents, and/or spouses who do not file a tax return must provide appropriate income information on the verification form.
Form 4506-T, Request for Transcript of Tax Return, can be used to request tax return transcripts, tax account information, W-2 information, 1099 information, verification of non-filing and record of account. Form 4506, Request for Copy of Tax Return, is now used only to request copies of tax returns. For more information regarding form 4506, please click here. NOTE: The Internal Revenue service (IRS) has indicated that they will not provide an IRS transcript until sometime after the income tax filing deadline. Click here to download a form that would accommodate the processing of files for students who have been selected for verification and have not and will not file a 2006 IRS Income Tax Return.
